Good nutrition is an essential during pregnancy as your baby’s health is dependent on it. Thus a healthy pregnancy diet is the key to a healthy pregnancy. Protein is an important nutrient contributing to muscle strength, breasts, uterus and bay tissues. It also enhances blood supply. During pregnancy an average of 65 g of protein is needed. Both lower and higher protein levels are detrimental during pregnancy. Food having a good protein composition is egg, pulses, meat products fish and cereals.

Fat requirement in the daily diet during pregnancy is 40 g which is equivalent to 5 teaspoonfuls of oil. One needs 300 mg/day of D HA during pregnancy. Omega-3 fatty acid is an important category of fat during pregnancy contributing to the brain development and vision of the growing baby.

Vitamins and minerals like folio acid is essential to build tissues and for overall development of red blood cells. Green leafy vegetables, citrus fruits, bread, beans and nuts are significant sources of folic acid. Vitamin B12 helps in the healthy development of brain and nervous system of the baby. Iron is another essential nutrient which regulates blood flow between the baby and mother.

Calcium is needed for strengthening the bones of the baby. A daily intake of 1000 mg per day during pregnancy should suffice. Rich source of calcium are milk, cottage cheese, cheese, green leafy vegetables and fish.

Fiber and fluids are significant to stay away from common pregnancy problems like constipation and piles. Eating lots of fresh fruits and vegetable, whole meal bread and cereals and brown rice improves your fiber intake. There are situations when the body does not get sufficient nutrition due problems like nausea and vomiting, heart burn, constipation, food aversion and some others. What Is Dyspareunia?

Dyspareunia is a general term for painful sexual intercourse for women. Dyspareunia is not a single condition but a syndrome, that is, a set of symptoms. It’s estimated that 50% of women experience painful intercourse at some time. Dyspareunia is not serious, and a number of treatments are available.

What Are the Symptoms?

Each woman has her own set of symptoms. Pain, of course, is the chief symptom. The pain occurs just before, during or after intercourse. Some women feel the pain on the surface or just inside the vagina (superficial pain). Others feel the pain down inside the pelvis (deep pain). Women have described the pain as cramping, burning and sharp.

When Should a Woman See Her Doctor?

Although widespread, painful intercourse is not normal. The pain usually signals a serious underlying medical or physical problem. A woman should see her doctor when the pain reoccurs and interferes with a normal sex life.

What Causes Dyspareunia?

Sources of superficial pain include:
• Vaginal dryness because of too little foreplay; some medications, such as antidepressants and certain birth control pills; or a drop in estrogen levels after childbirth or menopause
• Vaginismus, an involuntary tightening of the vagina muscles
• Damage or injury to the genital area from an accident, certain surgeries or a congenital (inborn) abnormality
• An infection in the genital area or urinary tract
• A skin irritation in the genital area
• Vulvar vestibulitis, an unexplained stinging or burning around the opening of the vagina

Causes of deep pain include:
• An illness, such as pelvic inflammatory disease, cystitis (inflammation of the bladder) or irritable bowel syndrome
• An infection in the fallopian tubes, uterus, ovaries or cervix
• Radiation treatments or chemotherapy for cancer in the pelvic region
• Certain conditions, such as hemorrhoids, ovarian cysts or prolapsed uterus (when the uterus slides from its normal position into the vaginal canal)
• Adhesions (scar tissue) resulting from surgeries in the pelvic region, uterine fibroids (benign tumors in the uterus) or endometriosis (when cells in the lining of the uterus grow elsewhere in the body)

In some cases, emotional or psychological problems are at the root of the pain. More often, however, they contribute to the underlying medical or physical cause. Mental illness, stress, fear of getting pregnant or not getting pregnant, insecurity over one’s sexual performance and negative attitudes toward sex can add to the pain.

What Are the Treatments for Dyspareunia?

The first step, naturally, is to relieve the pain with nonsteroidal anti-inflammatory drugs, such as aspirin and ibuprofen. The second step is to attack the underlying cause of the pain with the appropriate medication or therapy, for example:
• Antibiotics or fungicides for infections
• Estrogen replacement therapy for vaginal dryness
• The Wurn technique to break down and remove scar tissue
• Kegel exercises to strengthen the pelvic floor muscles
• Sex counseling

Many of these treatments can be carried out at home. But women should always check with their doctor before beginning any medical treatment.

How Can Dyspareunia Be Prevented?

Although it can’t be totally prevented, the risk of suffering dyspareunia can be reduced by:
• Maintaining good hygiene and overall health
• Lubricating the vagina before sexual relations
• Prolonging foreplay for natural lubrication
• Trying different sexual positions
• Changing meds that cause vaginal dryness
• Protecting oneself from sexually transmitted diseases
• Avoiding deep penetration, especially if endometriosis is present
• Communicating openly with one’s sexual partner

Feds Deny Women’s Health Renewal

In a letter dated Dec. 12, the U.S. Department of Health & Human Services’ Centers for Medicare & Medicaid Services denied Texas’ application to renew the successful Women’s Health Program, citing the state’s impermissible narrowing of the provider base to exclude Planned Parenthood from any participation.

The WHP is a Medicaid-waiver program that offers low-income and uninsured women ages 18-44 who wouldn’t otherwise be eligible for Medicaid an opportunity, unless pregnant, to receive preventative health and family planning care. The program, devised by lawmakers in 2005, was conceived as a way to save the state money and to prevent unwanted pregnancies. It has so far done just that: In 2009, according to the state’s Health & Human Services Commission, the program averted 10,300 Medicaid-paid births, saving the state roughly $46 million in all funds. Indeed, the program is paid for by a 90-10 match from the feds – for every $1 the state kicks in, the feds throw in $9. In other words, it’s a great deal for Texas, especially when you consider that 56% of all Texas births are paid for by Medicaid; in 2009 alone, that cost the state $2.7 billion.

But the future of the program is now in jeopardy. At issue is new language crafted to eliminate Planned Parenthood from providing any WHP services. This is no small issue, given that in 2008 PP was the single largest provider of such services. But because some PP clinics provide legal abortion services – though none with tax money – lawmakers directed HHSC to ask the feds to reauthorize the program, but with new language that redefines the word “affiliate” in an effort to squeeze PP out of the loop. Because the WHP is a waiver program, it is up to the feds to consider whether the language fits with legal restrictions for funding under Title XIX, which funds Medicaid.

The prevailing wisdom coming from providers and health care advocates was that targeting and excluding a specific provider – in this case PP – based on its providing legally protected abortion services to private-pay clients would not pass legal muster. Indeed, in the Dec. 12 letter, the Centers for Medicare & Medicaid Services concludes that the new “affiliate” language does just that. The feds “will not approve the State’s request for authority to deviate from the requirements of [Medicaid] in order to restrict beneficiary choice of family planning providers,” CMS Director Cindy Mann wrote.

Although the initial WHP waiver contained a provision excluding abortion providers from participation, that measure was never enforced as a wedge to exclude PP. Last year, Sen. Robert Deuell, R-Greenville, asked Attorney General Greg Abbott if it would be legal to enforce that provision and to exclude PP; Abbott agreed that it would. As such, state health officials this spring began a process of rewriting the definition of the word “affiliate” in order to eliminate PP as a health care provider choice for women; under the new definition, being affiliated with an abortion provider means any connection – so PP clinics that don’t offer abortion care would nonetheless be banned since they are essentially members of the same umbrella nonprofit.

According to stats provided to the Health & Human Services Commission’s Medical Care Advisory Committee this summer by commission staff, almost 30% of then-current program clients – just less than 130,000 in May alone, according to HHSC – are served by PP clinics. Although Kay Ghahremani, deputy director of Medicaid policy development for HHSC, told the committee that 98% of clients live within 10 miles of another provider, she had no information about whether the remaining providers had the ability to pick up tens of thousands of clients that would lose their provider should the prohibition on PP stand (based solely on May enrollment numbers, that would be roughly 39,000 women). According to an August report from HHSC, last year 80,430 women accessed WHP services at family-planning clinics, including PPs, while just over 9,000 women were seen at federally qualified health centers, one-stop medical shops that Deuell routinely champions as being the best place for women in need of preventative health and family-planning services and a replacement for the women’s health care system that relies so heavily on PP for services.

Seeing the WHP crash and burn would not be good for Texas women. With the number of enrollees increasing significantly each year – from a total of 91,683 clients served in 2007 to 183,537 in 2010, and expected to top that this year – the program is both a health care and financial success. And with the Legislature’s draconian cuts to the state’s family-planning budget for the current biennium – stripping roughly two-thirds of the budget historically used to provide low-income and uninsured women with access to gynecological exams, cancer screenings, and birth control, among other services – the WHP was seen by many as a light in an otherwise dark landscape, a potential way for women cut from the traditional family-planning program to obtain much-needed services. According to a 2008 Guttmacher Institute report, approximately 1.5 million women in Texas are in need of these health services; before the family planning cuts made in April, the state was on average serving roughly 15% of those women. With the budget cuts in place, tens of thousands of women stand to lose access to health care. Should the WHP dissolve, potentially nearly 200,000 will also lose coverage.

According to the CMS letter, the agency remains open to working with Texas to ensure that the WHP will be renewed, and has agreed to allow the program to continue through the end of March 2012 – it was set to expire, pending renewal, on Dec. 31. Whether Texas will be willing to compromise on the “affiliate” language in order to see the program live on is unclear. According to HHSC spokeswoman Stephanie Goodman, the agency is disappointed by the CMS decision, “which is inconsistent with federal law that gives states the authority to establish qualifications for Medicaid providers,” she wrote in an email. “HHSC will continue to work with CMS to continue this important program and enforce the state’s right to establish provider qualifications for the program that reflect the values of our state.”

Indeed, it seems that Gov. Rick Perry agrees that the state should be able to evict from the program any provider with which lawmakers have political differences, regardless of that provider’s ability to provide the medical services sought. According to a statement released last week, Perry apparently agrees that by denying women their choice of providers, the state is helping to protect unborn children. “We are committed to protecting life in Texas, and state law prohibits giving state dollars to abortion providers and affiliates – a fact the Obama Administration ignores,” he said. “I strongly urge the administration to do the right thing and grant this waiver, so Texas women can access critical preventative health services, including breast and cervical cancer screenings, rather than making them pay the price for its pro-abortion agenda.”

The news did not sit well with other lawmakers, including San Antonio Democratic Rep. Mike Villarreal, who has been an advocate for the WHP and who hopes that the state will work with CMS to ensure the program lives on. “Instead of playing politics with women’s health, state leaders need to use this time to find a way to continue the critical health services that Texas women rely on,” he said.

One thing that can surely destroy a woman’s sense of feeling desirable and sensual, is to have feminine odor. Since this malady plagues so many women, let’s look at some methods of feminine odor control.

If feminine odor already exists, the first thing you want to do is have a gynecological exam to rule out any vaginal infection, as vaginal infections definitely cause odor. If the result of your exam is positive for infection you will be given medicine to kill the unfriendly bacteria or yeast causing it; however, caution must be taken so that the infection does not reoccur. The methods used to prevent re-infection are, for the most part, the same for preventing feminine odor, so let’s look at these now.

Hygiene:

The most obvious feminine odor control method is to have proper hygiene. If need be, wash more than once a day but do not use harsh soap. Unscented natural soaps are the best, and certainly never use antibacterial soaps as they are too chemically laden and can very easily irritate the sensitive tissue around and in the vagina.

Soap also disrupts the vaginal pH. When this occurs the healthy vaginal flora cannot survive and hence disruptive organisms take over and cause infection and odor.

Natural Clothing:

Cotton and wool are not only natural fabrics, but they breathe, which means they allow airflow. Synthetic materials such as nylon and polyester do not breathe, and consequentially hold in moisture; and constant moisture breeds bacteria and yeast.

Now, just as when you perspire under your arms and you get an underarm odor, the same happens when you wear nylon panties or pantyhose, or polyester slacks — the vaginal area perspires; it can’t get air because of the suffocating material around it; and it begins to smell. Therefore, always wear cotton panties and try not to wear synthetic fabrics such as pantyhose or polyester slacks.

Air It Out:

There was a time in our history when women didn’t wear panties at all. I know going without panties might be a shock to some ladies, but unless you feel it is necessary, at least don’t wear panties to bed; let it air out.

Also consider thigh-high nylons instead of pantyhose.

Ditch the Douche:

Many women douche for the simple reason that they want to feel clean, especially after their period or after intercourse. Some women also douche if they notice a discharge, though some discharge is normal.

The trouble with douching, however, is that it can cause the very feminine odor that women are trying to eliminate. Douching is actually counter-productive and is certainly not a feminine odor control method.

When using a commercial douche, the chemicals in the product badly disrupt the vaginal pH. Friendly vaginal flora die because of this and then harmful bacteria take over, which again causes feminine odor.

Another issue with douching is that the force of the douching fluid can push existing bad bacteria up through the cervix, causing pelvic inflammatory disease; and pelvic inflammatory disease also emits odor.

In summation, the best means of feminine odor control is to wash regularly with a gentle, unscented soap; wear cotton panties and avoid synthetic fabrics in that area; sleep without panties and allow the vagina to air out; and do not use commercial douches.

Endometriosis and Infertility

Endometriosis is a condition in which cells from tissue inside the uterus (womb) grow in places outside the uterus. These cells settle on or in the fallopian tubes or ovaries, and sometimes on or in other organs in the pelvic area, including the abdomen and bladder. The cells clump up, become inflamed and eventually develop into adhesions (scars), which cause pain. In some cases, the condition leads to infertility, known as endometriosis infertility.

The cause of endometriosis is not yet known. Some women may be born with this abnormality. Some women may be born with a tendency to get endometriosis during their reproductive years. Medical researchers have suggested that during menstruation, menstrual fluid backs up through the fallopian tubes into the abdomen where cells from the fluid become embedded and begin to grow. There are a number of other theories for its prevalence.

Studies have shown that between 5% and 10% of all women have endometriosis, but most of these women are fertile. Some 30% to 40% of women with endometriosis are infertile. In most cases, endometriosis infertility is temporary. Women with endometriosis do have children. They just have more difficulty getting pregnant than other women have.

In most cases, it’s difficult to pinpoint why a woman with endometriosis has trouble getting pregnant. Some researchers think that infertility and delayed pregnancy might make a woman susceptible to developing endometriosis, rather than endometriosis causing infertility. Some researchers believe there are multiple causes of endometriosis infertility and that each woman who suffers this disorder has her own set of contributing factors.

Studies have shown that the likelihood of a woman suffering endometriosis infertility is directly related to the severity of her condition. Most cases of endometriosis are mild, with little or no pain. Nevertheless, woman with mild endometriosis take longer to get pregnant than women in general do. Women with moderate to severe endometriosis have even more difficulty getting pregnant than women with mild endometriosis have.

If severe enough, endometriosis can damage one or more of the woman’s reproductive organs. As the condition worsens, the adhesions formed from the clumps of cells grow thicker and larger in or around the reproductive organs. In general, adhesions that stick to or push against a reproductive organ deform the structure of that organ and impact its normal function. This contact, medical researchers believe, contributes to endometriosis infertility.

For example, adhesions in the fallopian tubes can nudge the tubes out of their normal position, thus preventing the egg from the ovary from entering the tube. Or the adhesions might block the inside of the tubes, keeping the egg from traveling to the womb. Thick adhesions on or in the ovaries can form painful cysts and sometimes prevent the egg from escaping. They can also reduce the number of eggs in the ovaries and damage remaining eggs. And inflammation in the pelvis caused by adhesions can trigger the production of certain cells that attack and shorten the life spam of the sperm.

Common treatments for endometriosis infertility include drugs to control pain and to stop the endometriosis from getting worse and surgery to remove the adhesions. But surgery often creates its own adhesions. The Wurn technique, a hands-on procedure used by physical therapists, has proven effective in breaking down and eliminating adhesions and, in many cases, restoring fertility without drugs or surgery.
